Composition
Each capsule of Phezam contains two active pharmaceutical ingredients:
- Piracetam – 400 mg
- Cinnarizine – 25 mg
Other ingredients may include lactose, talc, magnesium stearate, and gelatin (for capsule shell). This combination exerts both nootropic and vasodilatory effects.
Description
Phezam is a fixed-dose combination of a nootropic agent (piracetam) and a calcium channel blocker with antihistaminic activity (cinnarizine). The preparation is primarily used to improve cognitive function, enhance cerebral blood flow, and treat vestibular disorders such as dizziness and tinnitus.
Pharmacological Action
Piracetam facilitates neuronal metabolism and enhances neuroplasticity, increasing oxygen utilization and improving microcirculation in the brain without causing vasodilation. Cinnarizine acts as a vasodilator and H1-antihistamine, reducing inner ear labyrinthine sensitivity, which helps manage balance disorders and vertigo.
Indications
- Chronic cerebral circulatory insufficiency (memory disorders, poor concentration, mood disturbances)
- Vertigo and dizziness of central and peripheral origin
- Meniere’s disease and related labyrinthine disorders
- Post-traumatic brain syndrome
- Prevention of motion sickness
Contraindications
- Hypersensitivity to piracetam, cinnarizine, or excipients
- Severe renal impairment (creatinine clearance < 20 ml/min)
- Huntington’s chorea
- Pregnancy and breastfeeding
Dosage and Administration
The recommended dose for adults is 1 to 2 capsules 2–3 times daily, depending on the condition and medical advice. Treatment duration may vary but is typically several weeks to months. Capsules should be taken after meals with water.
Precautions
- Use caution in patients with renal dysfunction; dose adjustment may be necessary.
- Phezam may cause somnolence. Avoid driving or operating heavy machinery until individual response is known.
- Long-term use in the elderly should be accompanied by periodic liver and kidney function tests.
Side Effects
- Gastrointestinal: nausea, abdominal pain, dyspepsia
- CNS: drowsiness, agitation, nervousness, headache
- Skin: rash, allergic reactions
- Other: weight gain (due to cinnarizine), fatigue
Most side effects are mild and transient. If symptoms persist, consult a physician.
Drug Interactions
Concomitant use with CNS depressants (alcohol, sedatives) may enhance sedative effects. Cinnarizine may potentiate the effects of antihypertensives and anticholinergic drugs. Inform your doctor of all medications being taken.
Overdose
Symptoms of overdose may include sedation, hypotension, tremors, and gastrointestinal upset. There is no specific antidote. Treatment is symptomatic and supportive, including gastric lavage and monitoring of vital signs.
Storage
Store in a dry place at a temperature below 25°C. Keep away from direct sunlight and out of reach of children. Do not use after the expiration date indicated on the packaging.
