Heptral (Ademetionine) Tablets 500 mg, #20 Solution
Composition
Each film-coated tablet contains 500 mg of ademetionine (S-adenosylmethionine). Excipients may include mannitol, sodium hydroxide, magnesium stearate, and enteric coating materials. The formulation ensures stability and bioavailability of the active compound.
Pharmacological Group
Heptral belongs to the class of hepatoprotectors and choleretic agents. It also exhibits antidepressant activity, making it a multifunctional agent particularly useful in hepatic and neuropsychiatric conditions.
Indications
- Intrahepatic cholestasis
- Chronic liver disease (cirrhosis, hepatitis, steatosis, alcoholic liver injury)
- Drug-induced hepatotoxicity
- Adjunct treatment of depressive disorders (especially in SSRI-resistant cases)
Pharmacological Effects
Ademetionine participates in transmethylation and transsulfuration pathways. It supports hepatic detoxification by increasing intracellular glutathione and enhancing bile flow. In the CNS, it increases the synthesis of monoamine neurotransmitters like serotonin, dopamine, and norepinephrine, contributing to its mood-elevating effects.
Mechanism of Action
Ademetionine acts as a methyl donor in numerous metabolic reactions. In liver cells, it promotes membrane fluidity, bile acid transport, and hepatocyte regeneration. In the brain, it contributes to phospholipid synthesis and neurotransmitter metabolism.
Dosage and Administration
- Standard Dose: 800–1600 mg/day, typically divided into 2–4 doses
- Administration: On an empty stomach, at least 30 minutes before meals
- Note: Do not crush or chew the tablets; swallow whole with water
Contraindications
- Known hypersensitivity to ademetionine or excipients
- Children and adolescents under 18 years
- Use with caution in pregnancy and lactation; only under medical supervision
Precautions
- Use caution in patients with bipolar disorder due to risk of inducing mania
- Patients with Parkinson’s disease may experience symptom exacerbation
- Monitor liver function during long-term therapy
Side Effects
- Common: Nausea, abdominal discomfort, dry mouth, insomnia, dizziness
- Rare: Rash, allergic reactions, irritability, anxiety
- Severe (rare): Anaphylaxis, angioedema (seek emergency care)
Drug Interactions
- Use caution with MAO inhibitors, SSRIs, and tricyclic antidepressants—risk of serotonin syndrome
- May potentiate the effects of levodopa in Parkinson’s patients
Pregnancy and Lactation
Animal studies show no teratogenic effect; however, due to lack of well-controlled human studies, use during pregnancy and breastfeeding should be limited to cases where benefit outweighs risk.
Overdose
Symptoms may include agitation, insomnia, nausea, or gastrointestinal upset. Management is supportive and symptomatic. Seek immediate medical help if an overdose is suspected.
Storage
Store at room temperature (15–25°C), protected from moisture and direct light. Do not use after the expiry date. Keep out of reach of children.
