Mildronate (Meldonium) Ampoules 100 mg/ml, 5 ml, #10
Mildronate, also known by its active ingredient meldonium, is a cardiovascular medication designed to improve energy metabolism in ischemic tissues. This injectable form is primarily used for the treatment of heart and circulatory disorders, as well as physical and mental exhaustion caused by chronic conditions or high stress.
Composition
Each ampoule (5 ml) contains:
- Meldonium dihydrate – 100 mg/ml
- Sodium metabisulfite (stabilizer)
- Water for injection (solvent)
Pharmacological Group
Cardioprotective agent; anti-ischemic, cytoprotective.
Mechanism of Action
Meldonium inhibits gamma-butyrobetaine hydroxylase, decreasing carnitine biosynthesis and limiting fatty acid oxidation in mitochondria. This helps reduce oxygen consumption in ischemic tissues and shifts energy metabolism toward more favorable glucose oxidation, enhancing ATP production. It also improves endothelial function and cerebral circulation.
Therapeutic Indications
- Chronic ischemic heart disease (angina pectoris, post-infarction rehabilitation)
- Chronic heart failure
- Cardiomyopathy
- Cerebrovascular insufficiency
- Reduced work capacity and physical overstrain
- Adjunct in recovery after surgery or severe infections
Dosage and Administration
Intravenous administration only. Standard dosing:
- Cardiovascular disease: 500–1000 mg daily IV, once or in divided doses
- Physical/mental fatigue: 500 mg IV once daily
- Duration: Typically 10–14 days; course may be repeated every 2–3 months
Administer slowly; infusion may be diluted with 0.9% NaCl if required.
Contraindications
- Hypersensitivity to meldonium or excipients
- Severe hepatic or renal dysfunction
- Increased intracranial pressure
- Pregnancy and lactation (unless benefit outweighs risk)
Precautions
- Monitor renal and hepatic function during prolonged use
- Use with caution in patients with hypotension
- Not recommended in children under 18 (insufficient data)
Possible Side Effects
- Allergic skin reactions (rash, itching)
- Headache, dizziness
- Tachycardia, palpitations
- Gastrointestinal issues (nausea, upset stomach)
- Fluctuations in blood pressure
Overdose
No cases of life-threatening overdose reported. Symptoms may include hypotension, dizziness, or tachycardia. Symptomatic treatment recommended.
Drug Interactions
- Can be combined with antianginal, antiarrhythmic, and anticoagulant therapies
- Potentiates effects of nitrates and antihypertensives
- May intensify CNS stimulant effects when used concurrently

- Store below 25°C
- Protect from light and moisture
- Keep out of reach of children
Packaging
10 × 5 ml ampoules per pack. Clear, colorless sterile solution.
Shelf Life
2 years from the date of manufacture. Do not use after the expiry date printed on the box.
