Composition

Each gram of Iruxol ointment contains:

  • Collagenase – 10,000 units
  • Chloramphenicol – 100,000 units

Inactive ingredients include petrolatum, sorbitan sesquioleate, and lanolin alcohols.

Therapeutic Indications

Iruxol is indicated for enzymatic debridement of chronic dermal ulcers, necrotic wounds, and severe burns. The collagenase component facilitates the removal of necrotic tissue by selectively breaking down denatured collagen, thus promoting natural wound healing. Chloramphenicol provides a broad-spectrum antibacterial effect, helping to prevent secondary infections.

Pharmacological Properties

Collagenase is a proteolytic enzyme that hydrolyzes collagen in necrotic tissues, facilitating the breakdown and removal of eschar without harming healthy tissue. Chloramphenicol inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit. This dual-action mechanism aids both in wound cleaning and infection control.

Dosage and Administration

  • Cleanse the wound thoroughly and remove loose debris.
  • Apply a thin layer of Iruxol directly to the necrotic tissue once or twice daily.
  • The wound should be covered with a moist dressing.
  • Treatment should continue until satisfactory debridement has been achieved.
  • Wash hands before and after application.

Contraindications

  • Hypersensitivity to collagenase, chloramphenicol, or any excipients in the formulation
  • Presence of systemic infection or local infection at the wound site not appropriately managed
  • Concurrent use with silver- or iodine-based antiseptics (e.g., silver sulfadiazine) due to enzyme inactivation

Warnings and Precautions

  • Topical use only. Avoid contact with eyes, mucosa, and healthy surrounding skin.
  • Use with caution in patients with blood dyscrasias due to the presence of chloramphenicol, which has been associated with rare but serious adverse hematologic effects including aplastic anemia.
  • Limit duration of use and monitor for local and systemic adverse reactions.

Interactions

Collagenase activity is inhibited by heavy metal ions and antiseptics such as silver, iodine, or hydrogen peroxide. Do not use Iruxol concurrently with these substances.

Effects on Ability to Drive and Use Machines
