Description

Sodio Bicarbonato Monico is an alkalinizing agent used to treat metabolic acidosis and related conditions. It helps restore the body’s pH balance by neutralizing excess acid in the blood. The active component, sodium bicarbonate, is a white, crystalline, water-soluble compound with a mildly alkaline taste.

Indications

• Metabolic acidosis in severe renal disease
• Diabetic ketoacidosis
• Cardiac arrest and lactic acidosis
• Shock or severe dehydration
• Hemolytic reactions requiring urine alkalinization
• Poisonings by salicylates, methanol, or barbiturates
• Alkalinization during extracorporeal circulation

Contraindications

• Known hypersensitivity to sodium bicarbonate or any component
• Metabolic or respiratory alkalosis
• Hypocalcemia (risk of tetany)
• Hypochloremia
• Patients with sodium-restricted diets (e.g., severe hypertension, congestive heart failure)

Dosage and Administration

• Administer by slow intravenous injection
• Usual dose: 2–5 mEq/kg of body weight, depending on the severity of acidosis and blood pH
• Dosage should be adjusted based on blood gas and electrolyte monitoring
• Do not administer rapidly to avoid risk of hypernatremia or alkalosis

Side Effects

• Hypernatremia
• Metabolic alkalosis
• Hypokalemia
• Fluid retention and overload (edema)
• Muscle spasms, tetany, or seizures (from rapid pH shift)
• Hypertension or increased intracranial pressure in rare cases

Warnings and Precautions

• Use with caution in patients with heart failure, renal insufficiency, or hypertension
• Continuous monitoring of arterial blood gases, serum electrolytes, and renal function is essential during prolonged therapy
• May lead to hypokalemia—monitor potassium levels carefully
• Use with caution during pregnancy or lactation—consult a physician

Drug Interactions

• Enhances toxicity of cardiac glycosides (e.g., digoxin)
• May reduce serum lithium levels due to increased renal excretion
• Can increase risk of sodium retention with corticosteroids or corticotropin
• Amphotericin B may increase risk of hypokalemia when used concurrently

Overdose

Symptoms of overdose may include confusion, tremors, muscle twitching, hyperreflexia, paresthesias, tetany, irregular heart rhythm, or seizures. Immediate treatment includes discontinuation of sodium bicarbonate, correction of fluid and electrolyte imbalances, and symptomatic management.

Storage

• Store at 15–25°C in a dry, dark place
• Do not freeze
• Keep out of reach of children
• Inspect ampoules before use—do not use if solution is cloudy or contains particulate matter
