Composition
Each 1 ml ampoule contains:
- Testosterone Propionate – 50 mg
- Excipients: Benzyl Alcohol (preservative), Benzyl Benzoate (solubilizer), Refined Peanut Oil (vehicle)
Indications
Testosterone Propionate is indicated for testosterone replacement therapy in males with confirmed testosterone deficiency due to congenital or acquired conditions, including:
- Primary hypogonadism (testicular failure due to cryptorchidism, bilateral torsion, orchitis, vanishing testis syndrome, or orchiectomy)
- Hypogonadotropic hypogonadism (congenital or acquired)
- Delayed puberty in adolescent males (under specialized supervision)
- Inoperable metastatic breast cancer in females (as palliative treatment)
Mechanism of Action
Testosterone Propionate is a short-acting ester of the endogenous androgen testosterone. It acts via androgen receptors to stimulate protein synthesis, muscle growth, erythropoiesis, libido, and the development/maintenance of male secondary sexual characteristics. It also modulates luteinizing hormone (LH) and follicle-stimulating hormone (FSH) through negative feedback on the hypothalamic-pituitary-gonadal axis.
Dosage and Administration
- Route: Deep intramuscular injection into the gluteal muscle
- Typical adult dosage: 25–50 mg every 2–3 days
- Adjustment: Based on serum testosterone levels and clinical response
- Monitoring: Evaluate testosterone levels, hematocrit, liver function, and PSA regularly
Contraindications
- Known or suspected carcinoma of the prostate or male breast
- Hypersensitivity to testosterone or formulation excipients
- Women who are pregnant or may become pregnant (teratogenic risk)
- Severe hepatic, cardiac, or renal impairment (due to fluid retention risk)
Warnings and Precautions
- Monitor hematocrit (risk of polycythemia), especially in elderly patients
- Use with caution in patients with sleep apnea, benign prostatic hyperplasia (BPH), or hypertension
- May increase risk of prostate hypertrophy or prostate cancer – monitor PSA levels
- May impair fertility due to suppression of gonadotropins
Adverse Reactions
- Local: Pain, redness, or swelling at injection site
- Systemic: Gynecomastia, oily skin, acne, increased libido or aggression, fluid retention
- Endocrine: Suppression of spermatogenesis, alterations in lipid profile, polycythemia
Drug Interactions
- Oral anticoagulants: Increased anticoagulant effect – monitor INR
- Insulin or antidiabetics: Enhanced hypoglycemic response
- Glucocorticoids: Increased risk of fluid retention and edema
Overdose
Acute overdose is unlikely to cause life-threatening events. In case of excessive dosing, symptoms may include increased hematocrit, mood changes, or libido fluctuations. Therapy should be interrupted and resumed at a lower dose after clinical assessment.
