Mildronate (meldonium) is a metabolic modulator that improves cellular energy metabolism, particularly in ischemic tissues. It is widely used in cardiology and neurology to treat a range of conditions related to oxygen deprivation and circulatory dysfunction.
Composition
- Active ingredient: Meldonium dihydrate – 500 mg per capsule
- Excipients: Potato starch, gelatin, magnesium stearate
- Capsule shell: Gelatin, titanium dioxide (E171), indigo carmine (E132)
Each box contains 60 hard gelatin capsules.

Mechanism of Action
Meldonium inhibits carnitine biosynthesis, which decreases fatty acid oxidation and shifts metabolism toward glucose utilization—a more oxygen-efficient pathway. This effect helps protect ischemic cells and tissues from hypoxic damage, increases endurance, and improves recovery post-stress or ischemia.
Therapeutic Indications
- Chronic ischemic heart disease (angina, post-myocardial infarction)
- Heart failure and cardiomyopathy
- Cerebrovascular disorders (improved cognitive function, post-stroke rehabilitation)
- Physical and mental overexertion, including in athletes
- Alcohol withdrawal syndrome (as an adjuvant)
- Peripheral vascular diseases
Dosage and Administration
- Standard dose: 500–1000 mg daily, divided into 1–2 doses
- Maximum dose: 2000 mg/day
- Take in the morning to avoid potential insomnia
- Course duration: 4–6 weeks, repeatable every 2–3 months as needed
Contraindications
- Hypersensitivity to meldonium or any excipients
- Severe hepatic or renal dysfunction
- Increased intracranial pressure
- Pregnancy and lactation (insufficient safety data)
- Age under 18 years (not recommended)
Precautions
- Monitor liver and kidney function during prolonged use
- Use with caution in patients with hypotension
- Not intended for continuous use—intermittent courses recommended
Drug Interactions
- May enhance effects of:
- Antianginal agents (e.g. nitroglycerin)
- Beta-blockers
- Calcium channel blockers (e.g. nifedipine)
- Antihypertensive and antiarrhythmic medications
- May interact with anticoagulants, antiplatelets, and insulin/sulfonylureas
Side Effects
- Gastrointestinal upset (nausea, indigestion)
- Allergic skin reactions (rash, itching, urticaria)
- Fluctuations in blood pressure (usually mild hypotension)
- Tachycardia, palpitations
- Rare: agitation or dizziness
Overdose
No specific antidote is available. In case of overdose, supportive and symptomatic therapy should be provided. Contact emergency medical services if serious symptoms develop.

Shelf Life
4 years from the date of manufacture. Do not use after the expiry date.
